About the GREAT Health Program

Georgia is investing in the future of rural healthcare.

Beginning in 2026, and over a five-year period, the GREAT Health Program will provide funding to support projects that strengthen rural healthcare across the state. 

Funding opportunities will be open for applications from eligible healthcare entities offering care in specified areas of Georgia. Twenty-nine projects are being developed around the 5 main goals of the program, known as the program’s initiatives.

At its core, this program supports value-based care for Georgia's rural communities, helping providers improve patient outcomes based on quality and results.

The Georgia Department of Community Health will manage and award the federal funds. 

Looking Ahead

This initiative will continue after the Rural Health Transformation Program funding has completed. DCH will go through an additional application process to become fully part of the AHEAD program in mid-2026. Rural Health Transformation dollars will be spent to ensure providers are ready and fully informed on participation before the model launches in full, which is expected to be 2029.
